Ambulatory Care Rotation

Rotation Description:

The Ambulatory Care rotation at Community Health Net is an optional elective for the pharmacy practice resident. This experience exposes the resident to the principles of ambulatory care in a federally qualified health care center. The resident will spend four consecutive weeks (with a minimum of 40 hours per week) in the setting to learn the principles of caring for patients in an ambulatory care clinic whose patient population includes a large proportion of homeless, uninsured and Medicaid patients. Excellent communication and interpersonal skills are vital in this setting. The resident must devise efficient strategies for accomplishing all required activities.

The resident's tasks for this experience include, but should not be limited to:

  • Comprehensive assessment of patient's medication therapy and recommending evidence based therapy and care plans. The      resident must consider all patients characteristics and costs.
  • Making evidence based recommendations for therapy as requested by the providers at Community Health Net.
  • Fielding drug information questions from the staff and providing well researched and thoughtfully written answers.
  • Patient Case presentation or Journal Club
  • In-service presentation on a drug therapy topic for the staff of Community Health Net Assist in the effective precepting of      3rd year LECOM Scholl of Pharmacy students.
  • Actively assist formulary development and maintenance by participating in the functions of the Pharmacy and Therapeutics      Committee.
  • Counsel, educate and answer questions posed by the patient's of Community Health Net.
  • Rotation Topic Discussions / Readings

    Common disease states and medications with which the resident will be expected to gain facility through literature review, topic discussion and direct patient care experience include:

  • Coronary Artery Disease
  • Diabetes Mellitus
  • Hyperlipidemia
  • Hypertension
  • Atrial fibrillation
  • Congestive Heart Failure
  • Osteoporosis
  • Pain management
