Ambulatory Care Rotation II

Rotation Description:
Ambulatory Care is a required learning experience for the pharmacy practice resident. This experience is designed to provide the resident with the opportunity to utilize their acquired clinical knowledge and skills to deliver appropriate patient care. The experience involves direct patient care by interacting and collaborating with physicians, pharmacy students, and medical students in an ambulatory care setting. This is a calendar month rotation in which the resident will spend a minimum of 40 hours per week. The resident is responsible for collaborating with other healthcare professionals to identify, prevent and resolve medication therapy issues for patients. Excellent communication and interpersonal skills are vital in this setting. The resident must devise efficient strategies for accomplishing the required activities.
The resident's tasks for this experience include, but should not be limited to:
Explain the concept of pharmaceutical care.
Explain the characteristics a professional must possess in order to provide
optimal patient care.
Effectively communicate drug information to health care professionals in a
timely manner to solve medication therapy issues and enhance patient care.
Comprehensive assessment of each drug therapy regimen on an ongoing basis and development of a drug therapy plan for each patient while considering each individual's previous pharmacologic recommendations, current health care status (e.g. organ function, medical history), personal health related goals, costs, etc.
Designing and modifying therapeutic regimens to enhance patient care.
Providing specific and accurate recommendations pertaining to drug therapy including, but not limited to dose adjustment, drug addition, drug deletion, and drug monitoring both verbally and in writing incorporating the principles of evidence based medicine and established guidelines whenever possible.
Preparing a medication-related problem list.
Identifying therapeutic goals for a patient.
Designing monitoring parameters for a therapeutic plan.
Following up on effects of implementing a therapeutic regimen.
Effectively and professionally communicating with all health care related professionals, patients and families in regard to medications, drug therapy regimen evaluations, formal recommendations, in-service topics, and other required communications.
Review current medical literature and provide drug information to team
members.
Documenting direct patient care activities in the medical record.
Participate in and facilitate a smoking cessation program.
Assist in the creation of protocols and other improvement processes to maximize
patient outcomes.
Present an inservice on a pharmacotherapy topic
Thoroughly analyze and discuss a recent journal article
Rotation Topic Discussions / Readings
Some of the disease states that the resident should gain experience through literature and guideline review, topic discussions, journal clubs, in-services, and direct patient care experience include:
Diabetes Mellitus
Hypertension
Hyperlipidemia
Osteoporosis
Asthma
GERD
The resident is expected to understand the pharmacotherapy related to these disease states as well as others encountered in this setting. The preceptor or designee will be available to the resident throughout the learning experience for consultation and topic discussions. Resident learning is predicated not only to the above responsibilities but also on acceptance of personal responsibility and dedication to direct patient care and team service. The resident will also be required to teach the pharmacy students who are on rotation as well as other assigned tasks.
